$75.00 application fee is due at the time of application. You may pay mail, drop off a check in the school office, or by using Tuition Express

Required medical and additional forms are due August 1st

All students must have on file by August 1st the following documentation.

Health Inventory Form

• Please note that part 1 is to be completed by parent and requires a parent signature.

• Part 2 to be completed by physician and requires signature.

  Lead Form

• Lead Form to be completed by parent and physician. Requires physician signature, address and date.

  Immunization Certificate

• Immunization Certificate to be completed and signed by physician.

  Emergency Card

• Emergency Card must include at least 1 emergency contact. Note if child has medical concerns a physician must sign.

Children with medical conditions should have the following forms completed by their pediatrician as applicable.

Our Primary method of monthly payment is Tuition Express. Please download the Tuition Express application and submit with documentation.
