  $75.00 (non-refundable) application fee is required to process the application.

You may drop off a check in the school office or in our secure mailbox.

Required medical and additional forms are due August 1st

All students must have on file by August 1st the following documentation.

Health Inventory Form

• Please note that part 1 is to be completed by the parent and requires a parent's signature.

• Part 2 is to be completed by a physician and requires a signature.

  Lead Form

• Lead Form to be completed by parent and physician. Requires physician's signature, address, and date.

  Immunization Certificate

• Immunization Certificate to be completed and signed by a physician.

  Emergency Card

• Emergency Cards must include at least one emergency contact. Note if the child has medical concerns, a physician must sign.
